Before joining politics, Dr. Balbir Singh retired as an assistant professor and is working as an eye surgeon in Patiala. He did MBBS from Panjab University, Chandigarh in 1979. Balbir Singh also participated in the JP movement in 1974 and the Indian anti-corruption movement in 2011. Dr. Balbir had joined Aam Aadmi Party before the 2014 general elections. Dr. Balbir Singh served as the acting convenor of the Aam Aadmi Party of Punjab after the party's national convenor Arvind Kejriwal apologized to Bikram Singh Majithia. Following the apology, party convenor and co-convenor Bhagwant Mann and Aman Arora both resigned from their posts and Balbir Singh was appointed as co-convenor and acting convenor of the party's Punjab unit.

At that time Dr. Balbir Singh stood second with 20 thousand 179 votes. In the 2022 assembly elections, Dr. Balbir was contesting against Mohit Mohindra, son of senior Congress leader and former minister Brahm Mohindra on Patiala Rural seat. In this election, Dr. Balbir Singh defeated Mohit by a huge margin of 53 thousand 474 votes. Dr. in AAP government. Balbir Singh was honored with the post of Minister of Health and Family Welfare, Medical Education and Research in January last year.
