Moon on Shiva's head: The moon is definitely mentioned in the description of Lord Shiva's form. In any idol or picture of Mahadev, it can be seen that he has Ganga on his head and a half-moon on his forehead.

Story of Shiva Purana
According to Shiva Purana, during the churning of the ocean, Lord Shiva drank the poison Halahal. Lord Shiva is called Neelkanth because he held this poison in his throat.
According to the story written in the Puranas, after drinking poison, Mahadev's body started burning and his brain became hot. Seeing this scene of Mahadev, all the gods and goddesses became worried.

In such a situation, Chandradev requested Mahadev to accept him. The moon is considered to provide coolness. The heat of fire is also cooled by the coolness of the moon.
For this reason, Chandradev and all others prayed to Lord Shiva to place the moon on his head. It is said that after accepting everyone's prayers, Mahadev wore the crescent on his head.

According to another legend, it is said that Moon had 27 wives but he loved only one wife named Rohini. The other wives were often unhappy about this.
One day when Chandrama's other wives could not control their emotions they told their father Daksha about this and expressed their anguish, after which Daksha cursed Chandrama to become Prajapati.

Due to the curse, the entire glory of the moon started diminishing and Chandradev started suffering from terrible diseases. Then Devarshi Narad suggested Chandradev to worship Lord Shiva.
Chandradev not only worshipped Lord Shiva but also immersed himself in his intense heat. Then Lord Shiva appeared and removed Chandra's curse and as a reward for Lord Chandra's penance, he placed him on his head.
