
Even after five years of marriage, the dream of becoming parents remains unfulfilled. It is extremely painful for any couple mentally and emotionally. One such complex and surprising case came to light recently, where a woman was unable to conceive despite all efforts. What was most confusing was that the initial medical tests done by the woman and her husband, which included routine blood work, basic hormonal profile, semen analysis and pelvic ultrasound, all came back completely normal. In medical language, when pregnancy does not occur despite all the primary reports being fine, it is put in the category of ‘Unexplained Infertility’, but in this case the truth turned out to be different.
When the couple approached a senior fertility and gynecology expert, the doctor studied the old files in detail. Basic ultrasound was able to clarify the external structure of the uterus and the position of the eggs in the ovaries, but the internal pathway of the entire process of fertilization could not be confirmed. The doctor suspected that the problem might be hidden at a microscopic level in the fallopian tubes or the inner lining of the uterus (endometrial cavity). Internal obstruction of the tubes or subtle disorders of the uterus are often not detected in normal sonography. To clear this doubt, the specialist decided to conduct a special test called HSG (Hysterosalpingography – HSG) along with diagnostic hysteroscopy.
When the woman’s HSG and hysteroscopy test was done, even the medical team itself was surprised to see the report. Actually, there was a subtle ‘silent blockage’ (invisible obstruction) present in the fallopian tubes of the woman, which was not visible in any normal test for the last 5 years. Apart from this, a very fine polyp and mild tubercular endometritis (mild effect of pelvic TB) were found inside the uterus, for which the woman had never experienced any external symptoms or pain. The fallopian tube is the place where the male sperm and the female egg combine to form an embryo. Due to partial closure of the tube and swelling in the inner surface of the uterus, the embryo was not able to be implanted in the uterus.
As soon as the cause became clear, the doctors immediately prepared an accurate treatment plan. First of all, through minimally invasive techniques of laparoscopy and hysteroscopy, the polyp present inside the uterus was removed and the blocked fallopian tube was opened through cannulation process. Along with this, a fixed course of antibiotic and anti-inflammatory therapy was started to eliminate pelvic infection. Within a few months after the internal environment of the uterus became completely healthy and normal, the woman conceived naturally, turning a long wait of 5 years into a happy family.
Fertility experts say that about 10 to 15 percent of couples in India struggle with infertility, a large portion of which remain troubled without any apparent reason. If pregnancy does not occur after a year of marriage (or after 6 months if above 35 years of age) despite regular attempts, do not rely only on basic tests.
