The political turmoil and conflict in Pakistan’s capital Islamabad appears to be moving towards a very dangerous and violent turn. The Islamabad administration has imposed unprecedented arrangements like health emergency ahead of the massive protest march proposed on 27 September 2026 demanding the release of former Prime Minister of Pakistan and founder of Pakistan Tehreek-e-Insaf (PTI) Imran Khan, lodged in Rawalpindi’s Adiala Jail. Major government and military hospitals in the capital have been given written instructions to remain on high alert to deal with any possible serious humanitarian crisis and ‘Mass Casualty Event’ i.e. large-scale casualties. The declassification of confidential internal administrative documents has sparked panic across civil society, human rights organizations and political circles across Pakistan over whether the government is preparing to use large-scale force against protesters on the streets of the capital on September 27.
All scheduled surgeries postponed indefinitely: Strict order to keep surgical departments ready
According to official documents and Health Ministry orders obtained by international investigative media outlet ‘Drop Site’, major health institutions of Islamabad have been given clear instructions to postpone all their routine and scheduled surgeries indefinitely with immediate effect. Surgical wings, trauma centers and emergency wards of hospitals have been ordered to be completely evacuated and prepared for emergency situations so that in case of possible firing, tear gas attacks or stampede, a large number of seriously injured patients can be treated immediately. The direct impact of the administrative orders has started becoming visible on the common citizens, where from Monday, the patients admitted for routine surgeries are being sent back home after being informed about the cancellation of the procedures.
All leaves of doctors and paramedical staff cancelled: Order to return to duty immediately to the personnel on leave
As part of emergency preparedness in hospitals, all sanctioned and non-sanctioned leaves of all doctors, surgeons, nurses and support staff have been canceled with immediate effect. The concerned health administrations have been instructed to immediately recall the medical staff who are already on leave or out of the city to duty by sending emergency messages. Blood banks have been asked to secure additional stock of all blood groups, stockpile emergency life-saving drugs and keep additional ventilators and ICU beds ready. Although the Shehbaz Sharif government and the Home Ministry have publicly denied that the alert was a planned crackdown specifically against PTI, apprehensions have deepened with internal documents clearly mentioning preparations for a ‘potential major humanitarian crisis’ and ‘widespread casualties’.
Crackdown intensified before Islamabad march of 27th September: Imran Khan’s three sisters in police custody
This medical emergency alert in the capital’s hospitals comes at a time when police, rangers and law enforcement agencies across Pakistan have launched a massive crackdown against PTI’s top leadership and workers. The administration has directly targeted family members of former Prime Minister Imran Khan to stop the march towards the capital. Police and security forces have detained Imran Khan’s three real sisters—Alima Khan, Noreen Niazi and Dr. Uzma Khan—along with their families. According to media reports, a total of eight members of Imran Khan’s family were detained within the last 24 hours. According to Alima Khan’s son Shershah Khan, the family’s younger children were released after several hours of rigorous interrogation, while Khan’s sisters are still in the custody of law enforcement agencies.
30-day detention under Maintenance of Public Order (MPO): PTI alleges repression
Imran Khan’s sister Alima Khan has been detained under Section 3 of Pakistan’s stringent ‘Maintenance of Public Order’ (MPO) law. Lahore Deputy Commissioner has issued a formal order to keep him in custody for 30 days, citing maintenance of law and order and public peace. Similar administrative detention orders have also been issued against Noreen Niazi and Dr. Uzma Khan. Blockade has been started by placing heavy containers on all the main roads, motorways and GT roads coming to Islamabad from Punjab and Khyber Pakhtunkhwa. On the other hand, PTI leaders say that despite these repressive steps of the government, illegal arrests and creating an atmosphere of fear in hospitals, on September 27, lakhs of supporters will march to Islamabad at any cost for the release of their leader and restoration of the Constitution, due to which there is a danger of a major conflict in the capital.
