To obtain the certificate the user has to register online. Registration can be done using this link https://crsorgi.gov.in/web/index.php/auth/signUp. To sign up, the user will have to provide information related to name, mobile number, email ID, time of birth or death and address.
What documents need to be provided
In case of home birth, the parents will have to submit a declaration form in the prescribed proforma. Along with this, address proof will also have to be given. For that, any one of the documents like water identity card, electricity bill, gas bill, water bill, phone bill, passport, ration card, Aadhar card, bank account can be given.
The family will not be responsible for providing information in the event of a hospital birth. For this, the duty in-charge of the institution will have to give the birth information to the concerned registrar.
In how many days will the documents have to be given?
Information about birth will have to be given within 21 days. If unable to do so, parents must contact the concerned registrar. For delay of more than 21 days and less than 30 days, late fee and information will have to be given in the prescribed proforma i.e. Form 1. If the delay is more than 30 days and less than 1 year, then Form 1, Non Availability Certificate i.e. Form 10, Late Fee, Affidavit and permission of the concerned authority has to be submitted. In case of delay of more than one year, Form 1, Form 10, late fee, affidavit and order from First Class Magistrate will have to be obtained.

Information about death at home will have to be given within 21 days. For this, the family members will have to provide information and proof of address of the deceased through declaration form, Form 2. In case of death in the hospital, the duty in-charge will have to take responsibility. The special thing is that information about death will have to be given within 21 days of the incident. If this time limit is exceeded, the concerned registrar will have to be contacted.
If there is a delay……….
