First of all you have to go to voterportal.eci.gov.in. After logging in, you will get the option of voter ID correction.

For name correction, you will have to provide necessary documents to prove that your name is correct.

Finally, apply for review. If everything goes well then you can expect the new voter card to reach your home in about 15 days.
