District Disabled Empowerment Officer Richa Gupta has said that the Artificial Limbs and Assistive Devices Scheme is being run online for the benefit of the disabled by the Disabled Empowerment Department, Uttar Pradesh. Under the scheme, disabled persons with 40 percent or more disability, who have not received equipment under any scheme of the Government of India/State Government in the last 03 years, can submit their online application on the website and submit the hard copy of the application form along with all attachments to the office of District Disability Empowerment Officer Development. Available on any working day in Building Room No. 16, Hardoi.
For this, the applicant needs to have passport photo showing disability, Aadhar card, disability certificate issued by the Chief Medical Officer (minimum 40 percent), income, caste, residence (issued by Tehsil/Village Head), disability related equipment provided by the Chief Medical Officer or a designated doctor.
